Case management through one point of entry provides and enhances clients' opportunities to access needed services and prevent crises and further complications.
The range of services included in case management is limitless. JFS' experienced case managers assists clients with accessing and maintaining a relationship with various service providers, scheduling services, maintaining quality and evaluating the need for services, handling financial, legal, housekeeping and other matters as well as advocating on client's behalf, watching over client's benefits and maintaining close control over health / in-home care services.
Through this program JFS offers a wide spectrum of support services including emergency financial assistance, transportation services, mental health / counseling services, donation program, etc.
